The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Responsibilities include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients, providers, and other team members is critical to this role.

Required Skills & Certifications:
- Active Registered Nurse license in New York
- ACLS/BLS and NIHSS certifications
- EPIC charting experience
- Minimum of 2 years of hospital, home health, or hospice care experience
Preferred Skills & Certifications:
- PALS and TNCC certifications
Special Considerations:
- Must be available for some holidays
- Must be available every other weekend
Scheduling:
- 3x12 variable shifts, may have opportunity for straight night shift
- November start date
