Job Title: Registered Dietitian Nutritionist (Remote TX, CST)
Duration: 12 months (possibility to extend or convert)
Location: Remote must reside in Texas (CST time zone)
Schedule: Monday Friday, 8:00 AM 5:00 PM CST
Training: 5 weeks (camera required) Start Date: October 27, 2025
We are seeking an experienced Registered Dietitian Nutritionist (RDN) to join our team. This position focuses on nutrition support for chronic condition management programs, including diabetes, COPD, CHF, and other disease-specific pathways.
The RDN will play a vital role in providing telephonic and digital nutrition education , developing individualized care plans, and collaborating within a team-based care model to improve member health outcomes.
Conduct comprehensive nutritional assessments and develop clinically indicated dietary plans for members with chronic conditions (e.g., diabetes, COPD, CHF, depression, anxiety).
Provide nutrition education and self-management support via telephonic and digital outreach.
Lead nutrition support for low-risk members and assist with moderate/high-risk cases in collaboration with RN Care Managers.
Partner with Care Managers, Support Coordinators, and Behavioral Health specialists to deliver integrated member care.
Monitor member progress, adjust care plans as needed, and document all interactions in electronic health records.
Participate in program development, data collection, and quality improvement initiatives for chronic condition programs.
Maintain compliance with all state, federal, and organizational standards.
Education: Bachelor's degree in Nutrition, Dietetics, or a health-related field (required).
Licensure: Current, active, unrestricted Registered Dietitian Nutritionist (RDN) license in Texas.
Experience: Minimum 2+ years of disease-specific education or nutrition management experience, preferably in chronic care, home health, or care management settings.
Technical Skills: Proficiency in Microsoft Office Suite and electronic health record (EHR) documentation systems.
Other: Must have reliable internet and be available during CST hours.
Experience in managed care, LTSS, chronic disease management, or telephonic case management.
Strong communication, organization, and time management skills.
Bilingual (English/Spanish) is a plus.
Timely completion of member assessments and documentation.
Quality and accuracy of dietary education and care plans.
Member engagement and satisfaction outcomes.
Collaboration and communication within the interdisciplinary team.
